| Description: | Visiting Nurse Association of Pottstown & Vicinity is currently seeking a full time Registered Professional Nurse - Primary Nurse to perform nursing services requiring the application of the biological, physical, and social sciences, and nursing skills in the care of the sick, the prevention of the disease, and the conservation of health. Primary RN functions under the supervision of Team Managers, and supervises the care plan when LPN`s and Home Health Aides provide services to the client. Also responsible to case manage and provide skilled nursing care to clients requiring intermittent professional nursing services in a community health setting which may include performing a complete health assessment, effectively analyzing situations, developing goals and Plans of Care, and advocating for the client in obtaining needed services. The Primary RN is also responsible to plan with family, physician and interdisciplinary team appropriate care that is feasible within the physical, financial, environmental, social and emotional resources of the family; observe, evaluate and report to the physician any changes in the client’s condition; review all medication and identify possible ineffective drug therapy or adverse reactions; and provide coordination of services within the guidelines of the Plan of Care with other disciplines and the infusion therapy. Also required to demonstrate cooperation with the Manager and staff colleagues by identifying the caseload for the day, adjusting schedules to accommodate work, keeping managers informed of major changes in clients, assessing client’s home situation, and mobilizing community resources for protected services to neglected children and the aged. REQUIREMENTS Graduation from a State accredited Nursing Program and valid license to practice in the State of Pennsylvania, as well as one year of med/surg, critical care or ED nursing experience required. Strong computer skills and familiarity with computerized documentation. Must have a valid Pennsylvania Driver`s license, automobile insurance, available automobile during working hours, ability to drive in all types of weather, and be able to stand, walk, sit, and lift without weight restriction.
