Holistic Palliative Care
Pottstown VNA’s Palliative Care program is a holistic approach of comfort care. It can be useful when the outcome of an illness is uncertain, when support is needed while continuing aggressive treatment, and for individuals who are not yet ready for hospice care.
Our staff can also provide symptom management consultations.
Who Can Benefit From Palliative Care?
- People with intense pain and other symptom management issues
- People reaching the end of treatment options
Compassionate Hospice Care
Hospice Care is the specialized comfort care provided to the terminally ill. It is a holistic, patient-centered approach, which includes physical, spiritual, and psychosocial care and support for both the patient and family.
Care is provided for a wide variety of disease processes in many different community settings such as homes, skilled care nursing facilities and hospitals. The Hospice Team is made up of caring and compassionate nurses, social workers, a spiritual counselor, home health caregivers, and dedicated volunteers, and includes the assistance of other Pottstown VNA services when necessary.
Pottstown VNA also provides bereavement support for 13 months after your loved one has passed away. We understand that grieving is a process, and you don’t have to go through it alone. Families have the option to speak one on one with our spiritual counselor, attend support group meetings, or participate in special events in memory of loved ones.
